ADD/ADHD Information
If you or your child's teacher thinks your child may have ADD/ADHD, there are some steps to be taken to make the correct diagnosis. Download and print the Teacher Initial Assessment Form and give it to your child's teachers, and also complete the Parent Initial Assessment Form. As soon as these forms are completed, send them in to your child's physician for review. Your physician will then contact you to schedule the appropriate appointment in order to assess your child's needs. A recent report card from school is also helpful in making the proper diagnosis.
Children who have been diagnosed with ADD/ADHD will require specialized care and treatment. Your child will be expected to have an ADD/ADHD recheck visit within four weeks after starting medication and then at least every three months thereafter. If your child's medication is changed or altered, your child should be seen within one month of that change. The links below may address some of the questions or concerns you may have about your child's behavior. Feel free to contact your physician with any new or continuing problems your child is having.
Medication refill requests cannot be filled if your child is overdue for a recheck. Refill requests may take up to 48 hours to process and will be filled by your child's primary care physician only. Prescriptions for stimulant medication are tightly regulated by the Federal Government. The prescription must be written, cannot be refilled, must be filled within 5 days of when it was written and quantities dispensed are limited. These prescriptions cannot be transmitted electronically to your pharmacy. Please contact our office for a medication refill at least 1 week before your child will run out of medication.
Arrange to either pick up the prescription or have it mailed to your home.
If your child is scheduled for an ADD/ADHD recheck, please print and complete the PARENT AND TEACHER Assessment Forms below and bring them with you to the visit. A recent report card from school should be provided as well.
A PARENT OR GUARDIAN MUST ACCOMPANY YOUR CHILD TO THE VISIT. IF A CHILD COMES TO OUR OFFICE FOR A RECHECK WITHOUT A PARENT OR GUARDIAN, THE VISIT WILL BE RESCHEDULED
